Welcome to the Summer Staff page.  Any full-time summer applicant will need the following 5 forms: 

• Staff Application form (either new staff or returning staff)

• an I/A Inventory form

• three Staff Reference forms

• a notarized Criminal History form (a notary public is found at most banks)

• and a Health form 

Online Application

You may fill out the Application and I/A Inventory form online, but you still need to print off and send in the other 3 forms (References, Criminal History & Health).
